In addition to completing the online application, you'll need to submit a photo (headshot), two recommendations submitted either through an Online Recommendation Form, or mail letters of recommendation to Ignite Louisville Selection Committee, Leadership Louisville Center, 732 W. Main St., Louisville, KY 40202. Finally, we will require a Supervisor Commitment Form.
